Just did your survey. Couple of recommendation to you:

1) There are some questions repeated.
2) Make this survey little shorter.
3) I do not like the platform you are using for this survey. Use Survey Monkey or Survey Gizmo.

Completed your useful but lengthy survey. This definitely did not take only 5 minutes. Hence, I would recommend to get a shorter version of this.
